Health Check for Campus Access

Welcome to the Daily Health screening, formerly referred to as the "Campus Access Form."

Throughout the pandemic, this form was used to help track who was coming onto campus, from employees to students engaged in labs. As we have started to bring more students back and we are a dedicated vaccinated campus, we are shifting the focus on this survey to our community health.

Our community health starts with you doing a health check each day before coming to campus.

RTC requires this health check to be completed by our students, faculty, and staff who will be working on campus. Please complete this health check each day you visit our campus.

We believe having a healthy campus starts with you doing a quick health check to confirm:

  • You have not been exposed to COVID-19. (If you are vaccinated and have been exposed, you may still come to campus, but monitor for symptoms.)
  • You are not awaiting test results for a COVID-19 test. (Stay home if you are sick/symptomatic. If you are vaccinated and do not have symptoms, you can still come to campus)
  • You do not have any COVID-19 symptoms that are unrelated to another known medical condition, for instance, allergy symptoms that resolve with allergy medication.

If you have tested positive for COVID-19, please visit rtc.edu/COVID to help provide us with some basic information about your COVID-19 case and to help us prevent further spread of COVID-19 on campus.
